PLEX Database Privacy Policy

Praxis is committed to maintaining the accuracy, security and privacy of personal information in accordance with applicable legislation. Consequently, Praxis has developed policies and implemented security measures to comply with its privacy obligations when collecting, using and disclosing personal information in its PLEX database.

What is personal information?

Personal information is any information about an identifiable individual, which may include, but is not limited to, your name, address, email address, telephone number, age, healthcare history, socioeconomic information, education, etc.

You will be able to decide the personal information with which you are comfortable sharing with Praxis.

Anyone who has an interest in Praxis’ activities, including persons with lived experience (PLEX) of spinal cord injury (SCI), their families, friends, community and advocacy organizations, clinicians, researchers, and innovators, can request to be included in the PLEX database.

Purposes for collecting your personal information

Praxis will collect your personal information for the purposes of developing a PLEX database to contact you to share updates and newsletters about Praxis’ work, and for the active and meaningful engagement and recruitment of PLEX for research studies, project-partnerships, focus groups, interviews, educational/professional development and similar activities or opportunities.

The personal information you provide to Praxis will only be used by authorized staff to fulfill the purpose for which it was originally collected, or for a use consistent with that purpose. Praxis will not disclose your information with external third parties, unless you authorize it or Praxis is required by law to do so.

Safeguards

Praxis has made reasonable security arrangements to protect your personal information against unauthorized access, collection, use, disclosure and disposal.

How long will Praxis use your personal information in the PLEX database?

Praxis will maintain your information in the PLEX database until you make a request to delete it.

Exercising your rights

You have the right to access:

  • Your personal information
  • Information about the ways in which your personal information has been disclosed
  • The names of the individuals and organizations to which your personal information has been disclosed.

To request access, correction, deletion or to make a complaint involving the use of your personal information, you must contact Praxis’ Privacy Officer at privacy@praxisinstitute.org. You may be required to prove your identity before Praxis gives you access to your personal information.

Praxis will provide requested personal information within 30 business days, unless a time extension is granted under the Personal Information Protection Act (PIPA).

If Praxis is authorized or required by PIPA to refuse access, Praxis will inform you in writing, stating the reasons for the refusal.

If you are not satisfied with the manner in which Praxis handled your request, you may contact the Office of the Information and Privacy Commission for British Columbia.
