You may provide link(s) to various contents, such as videos, audio recordings, images, and written text (“Content”). You may only provide content that you own and/or have the right to share. Praxis Spinal Cord Institute (“Praxis”) may remove the content that it deems inappropriate and/or violates this Copyright Policy.

You may not submit any content that includes, but is not limited to, the following:

• Infringes any third party’s copyrights or other rights (e.g., trademark, privacy rights, etc.);
• Is sexually explicit or promotes a sexual service;
• Is defamatory;
• Is harassing or abusive;
• Contains hateful or discriminatory speech;
• Promotes or supports violence, terror or hate groups;
• Exploits or endangers minors;
• Depicts or encourages self-harm or suicide;
• Depicts (1) unlawful real-world acts of extreme violence, or (2) animal cruelty or extreme violence towards animals;
• Promotes fraudulent or dubious money-making schemes, proposes an unlawful transaction, or uses deceptive marketing practices;
• Contains false or misleading claims about any health-related information that has a serious potential to cause public harm;
• Violates any applicable law.

Consent

I hereby confirm that I own all rights to use, release and share all contents submitted to Praxis Spinal Cord Institute (“Praxis”).

I hereby grant Praxis and its representatives all rights, which include but are not limited to reproduce, use, exhibit, display, broadcast, distribute, and create derivative works of the content in any media now known or later developed, for use in connection with promoting or publicizing the content in order to facilitate mutual learnings and promote the exchange of good practices in the area of spinal cord injury care, health and wellness.

I also grant Praxis permission to share the content with partner organizations and on digital media platforms for use in connection with Praxis-supported activities. This includes, without limitation, the right to publish the contentin all forms of media, including but not limited to print, online/digital, radio and television. Publications may include annual reports, fundraising materials, advertisements, promotional materials, proposals, requests for proposals, newsletters, educational webinars, website and social media posts, and any other publications as released by Praxis, partner organizations or external media.

I understand that Praxis cannot control unauthorized use of the content I provided once the content has been published.

I understand that I may revoke this consent at any time by notifying Praxis’ Privacy Officer at privacy@praxisinstitute.org in writing and that the withdrawal will only apply to continued and future use and disclosure of the content provided by me.

Waiver

I hereby waive the following rights:

1. Any right to inspect or approve the use of the content provided by me; and
2. Any right to royalties or other compensation of any kind arising from or related to the use of the content provided by me.

Indemnity

I hereby release, defend, indemnify and hold harmless Praxis and its agents, officers, directors, employees, affiliates, successors and assigns from and against any and all liability, claims, damages and demands of whatever kind or nature, either in law or in equity, which may arise from or related to the use of the content provided by me.

I am 19 years of age or older and I am competent to contract in my own name. I have read this document before signing below, and I fully understand the contents, meaning and impact of this consent, waiver, indemnity and release. This consent, waiver, indemnity and release is binding on me, my heirs, executors, administrators and assigns.

If above named is a minor (under age 19), also the consent of the parent/legal guardian must be obtained.
